Steps to Working Together
Reach out by completing my Chemistry Call Questionnaire. This is also the first step in scheduling an appointment.
STEP 1 – Complete Chemistry Call Questionnaire
Completing this questionnaire gives me initial information regarding you and your needs as well as helps me to determine if we would be a good fit in working together. I am happy to provide potential resources if I am unable to meet your needs.
Once submitted, you will be able to schedule a free 10-minute phone consultation.
STEP 2 – Submit Registration Packet
If it seems that we would be a good fit and our availability matches up, you will then receive a second email inviting you to complete a Registration Packet. This Registration Packet must be completed prior to scheduling an appointment.
STEP 3 – Schedule an Appointment
Upon completing the Registration Packet you will be redirected to my online scheduling site to schedule your first appointment.
You will also receive a confirmation email with instructions on setting up your account in my client portal so that we can communicate in a HIPAA compliant manner.
